Qualifications

Responsibilities

• Perform person-centered assessment and planning with residents to set goals.
• Coordinate services, referrals, and transitions across care teams and programs.
• Maintain timely documentation and follow-up, ensuring confidentiality and accuracy.
• Provide advocacy and resource navigation to support residents’ independence and stability.
• Monitor progress, address barriers, and support transition planning from program entry onward.
• Collaborate with Care Team, residential staff, external providers, and community resources.
