Job summary

MIRCI in Columbia, SC is seeking a Social Worker/Case Manager to provide intensive, person-centered case management for LEAD referrals, addressing substance use, mental health, housing, and other barriers to help participants achieve stability and recovery.

Under supervision, you will coordinate services, conduct screenings, develop service plans, advocate with partners, and maintain confidential records while supporting reintegration and harm reduction in the community.

Qualifications

• Master's degree in Social Work required.
• LMSW preferred, but not required.
• 2+ years related behavioral-health case management preferred.

Responsibilities

• Maintain weekly contact with participants through visits, calls, texts, or home visits.
• Complete initial and ongoing screenings to identify needs.
• Develop and update individualized service plans with goals.
• Coordinate referrals to treatment, housing, healthcare, and community resources.
• Monitor engagement, progress, and court orders; adjust services as needed.
• Provide crisis intervention and safety planning using harm-reduction approaches.
• Educate participants about recovery resources and LEAD program expectations.
• Advocate with RCSD staff, courts, providers, and community partners.
• Maintain confidential records of assessments, contacts, plans, and referrals.
• Participate in meetings, supervision, and program evaluation activities.
