• Ability to work with individuals and families across diverse backgrounds and settings.
• Strong organizational, verbal, and written communication skills.

Responsibilities

• Perform person-centered assessment and planning; develop Coordinated Individual Case Plans with measurable goals.
• Coordinate services, resources, referrals, and transition planning to support resident stability and independence.
• Maintain timely, confidential documentation and collaborate with interdisciplinary teams.
• Provide advocacy and navigate community resources; support resident progress toward outcomes.
• Manage caseload and ensure timely responses to urgent needs and transitions.
• Participate in Care Team meetings and maintain professional boundaries and accountability.
