Resources for Human Development in Delaware is seeking a compassionate Case Manager to support individuals on their journey toward recovery, independence, and community connection. You will partner with participants to identify goals and develop recovery plans.

This role emphasizes person-centered, community-based care with collaboration from a multidisciplinary team, travel within the community, and strict privacy and reporting standards.

Qualifications

• Bachelor's degree in Human Services or related field.
• Minimum of two years of relevant experience.
• Intermediate computer skills.
• Valid driver's license.
• Ability to pass Federal and State criminal background checks.

Responsibilities

• Partner with participants to identify strengths, needs, and goals.
• Develop and support individualized recovery and service plans.
• Advocate to help participants access essential services and resources.
• Work with a multidisciplinary team to provide holistic, person-centered care.
• Assist with recovery plan development and ongoing reviews with participants and supports.
• Provide community-based services focused on education, skill-building, and engagement.
• Build and maintain relationships with community agencies and service providers.
• Identify service gaps and recommend strategies to improve supports and outcomes.
• Complete assessments, documentation, and reports per agency requirements.
• Utilize collaborative documentation practices during treatment contacts.
• Participate in rotating on-call schedule, including crisis intervention support.
• Deliver at least 75% of treatment contacts within the community setting.
• Maintain confidentiality and HIPAA compliance.
• Perform other related duties as assigned.
