Job summary

Indiana Treatment Centers is seeking a compassionate Case Manager in Columbus, IN to coordinate services, connect clients with community resources, and advocate for their long-term recovery. You’ll work on a multidisciplinary team to support individuals navigating treatment and transition into recovery.

The role emphasizes trauma-informed care, strong documentation, and collaboration with housing, employment, and health resources to promote stability and independence.

Qualifications

• High school diploma or equivalent required.
• Minimum of one year of case management or related services in behavioral health.
• Knowledge of community resources and social service systems.
• Experience documenting services in an Electronic Health Record (EHR).
• Strong communication, organization, documentation, and follow-through skills.
• Ability to work in an interdisciplinary treatment team.
• Current CPR/First Aid certification or ability to obtain within 30 days of hire.

Responsibilities

• Provide comprehensive case management for clients with psychosocial needs.
• Coordinate discharge planning and transitions of care to support continuity of services.
• Facilitate interdisciplinary case reviews and communicate client progress and barriers.
• Assess client needs and identify barriers impacting treatment or recovery.
• Connect clients with housing, employment, transportation, insurance, and community resources.
• Assist clients with obtaining identification, benefits, and other essential resources.
• Build relationships with referral sources, community agencies, and partners.
• Advocate for clients while promoting accountability and recovery.
• Coordinate referrals and follow-up with outside providers for proper linkage.
• Maintain timely, accurate documentation in the Electronic Health Record (EHR).
• Participate in treatment team meetings, quality improvement, and compliance activities.
• Maintain professional boundaries and protect client confidentiality.
• Support a trauma-informed, recovery-focused treatment environment.
• Perform other duties as assigned.
