• Ensures delivery of Medicare Important Message (IMM) letters and related communications.
• Schedules appointments with PCPs for high-risk patients.
• Assists Care Management with administrative tasks (discharge paperwork, referrals, follow-ups).
• Documents work in EPIC and maintains notes and queues.
