Job summary

Elite Home Health & Hospice is seeking a compassionate Admissions Registered Nurse (RN) to guide patients and families through the admission process for home health and hospice care. You will perform initial assessments, educate families, coordinate with physicians and facilities, and ensure smooth transitions to services with dignity and compassion.

The role requires an active RN license with strong clinical assessment, communication, and customer service skills.

Qualifications

• Active, unrestricted RN license.
• Home Health and/or Hospice experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, relationship-building, and customer service skills.
• Ability to work independently while managing multiple priorities.

Responsibilities

• Conduct admission visits for home health and hospice patients.
• Complete comprehensive assessments to determine eligibility and care needs.
• Educate patients, families, and caregivers on services, plan of care, and expectations.
• Coordinate referrals and admissions with physicians, hospitals, facilities, and community partners.
• Obtain and process physician orders and required documentation.
• Establish initial plans of care and transition to services.
• Collaborate with interdisciplinary team to ensure timely patient care.
• Maintain accurate clinical documentation per agency, state, and accreditation rules.
• Serve as a trusted clinical resource for patients and families during admission.
