Advanced Home Health and Hospice is seeking a Home Health Admissions Nurse to facilitate patient admissions into our home health program, delivering compassionate, high-quality nursing care during transitions.

You will conduct thorough assessments, collaborate with the care team, complete paperwork, coordinate services, administer medications, and support patients and families while ensuring regulatory compliance and accurate documentation.

Qualifications

• Valid RN license in the state where work is performed.
• Minimum 2+ years nursing experience; home health preferred.
• Strong assessment and clinical judgment abilities.
• Excellent interpersonal and communication skills.
• Knowledge of home health regulatory requirements and documentation.
• Strong organizational and time-management skills; multitask.
• Valid driver's license and reliable transportation for visits.

Responsabilités

• Conduct thorough assessments of incoming patients to gather medical history and needs.
• Collaborate with physicians and the interdisciplinary team to plan care.
• Communicate with patients and families about the admissions process and services.
• Complete admission paperwork and obtain necessary documentation.
• Coordinate with home health disciplines to arrange assessments and services.
• Administer medications and treatments per physician orders.
• Monitor vital signs and report changes to the team.
• Provide emotional support to patients and families during admission.
• Ensure compliance with regulatory requirements and patient rights.
• Participate in team meetings to discuss care plans and progress.
• Perform other duties as assigned by nursing leadership.
