Elite Home Health & Hospice is seeking a compassionate Admissions RN (RN) to guide patients and families through the admission process for home health and hospice care. You will perform comprehensive assessments, educate about services and care plans, and coordinate a seamless transition to care with physicians, hospitals, and partners.

Qualificações

• Current, unrestricted RN license.
• Home Health and/or Hospice experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, relationship-building, and customer service skills.
• Ability to work independently while managing multiple priorities.
• Valid driver’s license, reliable transportation, and current automobile insurance.

Responsabilidades

• Conduct admission visits for home health and hospice patients.
• Complete comprehensive assessments to determine eligibility and care needs.
• Educate patients, families, and caregivers on services, plans of care, and treatment expectations.
• Coordinate referrals and admissions with physicians, hospitals, facilities, and community partners.
• Obtain and process physician orders and required documentation.
• Establish initial plans of care and facilitate transitions to services.
• Collaborate with interdisciplinary team to ensure timely patient care.
• Maintain accurate clinical documentation in line with requirements.
• Serve as a trusted clinical resource for patients and families during admission.
