Job summary

Home of Guiding Hands is seeking a part-time Community Support Professional to work one-on-one with adults with intellectual and developmental disabilities. You will provide individualized support in homes and community settings, helping people build life skills and pursue personal goals.

The role emphasizes independence, advocacy, ISP coordination, and documentation. If you enjoy flexible, meaningful community work and helping others participate in activities they value, this on-site position

Qualifications

• High school diploma or GED is required.
• Must be at least 18 years old.
• Background check under DOJ is required.
• Must have valid California driver's license with safe driving record.
• CPR/First Aid certification or willingness to obtain it.

Responsibilities

• Plan and conduct visits with assigned clients
• Help individuals access and participate in their community
• Support clients in working toward their ISP goals
• Work with clients to develop meaningful personal goals
• Advocate for the individuals you support
• Attend and participate in ISP meetings
• Communicate with family and support team
• Complete required documentation and reports
• Encourage independence, choice, and community involvement
