Job summary

COMMUNITY CARE HOME INC seeks a Direct Support Professional to assist individuals with physical and/or intellectual disabilities in daily living and community participation. You will implement the Individuals’ Support Plans, monitor health and safety, and collaborate with families and agencies to promote independence.

The role requires clear English communication, ability to work independently in field locations, and flexible hours including driving to assignments; a high school diploma or GED

Qualifications

• Must be able to perform First Aid/CPR and observe resident needs.
• Maintain confidentiality of information at all times.
• Communicate clearly in English; read and follow English instructions.
• Organize tasks, multi-task, and prioritize assignments.
• Learn and apply positive behavior modification methods.
• Facilitate groups and provide individual skill instruction.
• Write legibly in English in a clear, concise manner.

• Work independently with minimal supervision in field locations.
• Maintain professional interaction with individuals and co-workers.

Responsibilities

• Provides personal care and protective oversight and supervision.
• Provides services, supports, care and treatment and/or supervision as required.
• Participates in ISP development and implementation.
• Provides supervision as determined in ISP Plans.
• Adheres to regulatory requirements.
• Responsible for personal and financial transactions of individuals.
• Ensures the facility/home is safe, comfortable, and hazard-free.
• Provides coverage in emergency situations.
• Performs other duties as assigned.
• Assesses and documents instructional needs initially and ongoing.
• Ensures health, safety and welfare; promotes community inclusion.
• Provides assistance in self-help, socialization, and adaptive skills training.
• Implements behavioral support plans to reduce maladaptive behaviors.
• Assists with independent living skills such as hygiene, housekeeping, meals, transportation, shopping, communication, medication and money management.
• Provides instruction in rule compliance, attendance, task completion, and problem solving.
• Supports social interaction skills in workplace settings.
• Delivers participant-specific assistance, including personal care and medication administration.
• Implements all program plans to enhance social, emotional, cognitive, physical, and independent living skills.
• Completes and maintains daily/weekly/monthly records and reports.
• Collaborates with families and other service providers; participates in meetings.
