Job summary

COMMUNITY CARE HOME INC is seeking a Direct Support Professional to provide personal care, supervision, and support in daily living activities. You will implement the Individual’s Support Plan, maintain records, and collaborate with families and service providers.

The role requires strong English communication, basic computer skills, and ability to work with individuals with intellectual disabilities. Driving and community integration support are part of duties.

Qualifications

• High school diploma or GED required; professional license if applicable.
• Strong English reading, writing and verbal communication skills.
• Ability to drive and obtain clearances as per regulations.

Responsibilities

• Provide personal care, supervision and support as required.
• Participate in development and implementation of Individual’s Support Plan (ISP).
• Maintain accurate records and reports in a timely manner.
• Collaborate with family, agencies and service providers.
• Assist with community outings and transportation.
• Support independent living skills and daily activities.
• Follow health care recommendations and monitor safety.
• Ensure confidentiality and professional interaction with individuals.
