Job summary

Home of Guiding Hands in San Diego County is seeking a Community Support Professional to work with adults with developmental disabilities in the community and at home settings. You will provide individualized support, help participants build life skills, pursue personal goals, and increase independence through community activities and ISP-driven tasks.

This part-time, on-site role requires a valid California driver’s license, flexibility, and a compassionate approach; training and ongoing

Qualifications

• High school diploma or GED is required.
• Must be at least 18 years old.
• Experience supporting individuals with special needs is preferred but not required.
• Valid California driver's license with a safe driving record and at least two years of driving experience.
• Reliable, safe vehicle for transporting clients.
• CPR/First Aid certification or willingness to obtain it.
• Comfortable using email, text messaging, websites, and apps.
• Able to work independently and manage your schedule.
• DOJ background check clearance.

Responsibilities

• Plan and conduct visits with assigned clients.
• Help individuals access and participate in their community.
• Support clients in working toward their Individual Service Plan goals.
• Develop meaningful personal goals with clients.
• Advocate for the individuals you support.
• Attend ISP meetings.
• Communicate with family members and other support team members.
• Complete required documentation and reports.
• Encourage independence, choice, and community involvement.
