Job summary

Options in Community Living in Madison, WI seeks a Community Support Coordinator to coordinate and provide support to adults with IDD. You will work as part of a team, manage a small caseload, and maintain a flexible schedule within office hours 9-5 Monday through Friday, with occasional evenings and weekends.

Qualifications

• Must be 18 years or older.
• Valid driver's license, insured car and good driving record.
• High School or equivalent required.
• Successful clearance of background checks.
• One or more years' experience working with adults with IDD.

Responsibilities

• Develop and implement individualized support plans.
• Hire, train and supervise direct support staff.
• Provide direct support and personal care as needed.
• Convene and run regular household meetings.
• Assist people supported to manage personal finances.
• Maintain accurate and timely documentation of time spent and log notes.
• Coordinate medical care for people supported.
• Work cooperatively with family, friends and other service providers.
• Participation in agency on-call rotation (with additional compensation).
