Job summary

ReDiscover is seeking a Mental Health Case Manager for its CPRC in Lee's Summit, MO. You will manage a caseload of clients, delivering services in person or by phone to support independent living and symptom management.

You will collaborate with a multidisciplinary team, connect clients to community resources, and assist with transportation as needed. Requirements include a 4-year degree or equivalent experience, valid driver’s license, and strong communication skills, with familiarity in the KC

Qualifications

4 year degree or 4 years of equivalent experience in lieu of degree.
A valid automobile driver’s license, insurance and a safe vehicle for client transportation.
At least two years direct clinical experience with children/adolescents in mental health preferred.
Knowledge of Kansas City metro resources and ability to partner with community organizations.

Responsibilities

Maintain and interact with a caseload of clients in-person or by phone.
Facilitate achievement of long-range outcomes like symptom management, employment, and independent living.
Engage difficult-to-serve clients by locating resources and making connections.
Help clients overcome barriers by linking them to community or social services and educational programs.
Assist clients with transportation barriers as identified in treatment plans.
