Job summary

The Guidance Center in Cumberland County, New Jersey, seeks a PATH Case Manager / Housing Specialist to deliver community-based case management, housing navigation, advocacy, and recovery support to individuals experiencing homelessness and serious mental illness.

You will help clients access housing, healthcare, benefits, and community resources while coordinating with partners and applying evidence-based engagement strategies.

Qualifications

• Bachelor's degree in Social Work or related human services field.
• Minimum two years of experience in case management, social services, behavioral health or homelessness services.

Responsibilities

• Provide community-based case management, outreach, and care coordination.
• Develop and monitor Individual Recovery Plans and client-centered care plans.
• Assist clients with obtaining stable housing and access to benefits.
• Coordinate with mental health, medical, and social service providers.
• Advocate for entitlement programs and navigate public benefits.
• Document services and participate in multidisciplinary team meetings.
