Porter-Starke Services, Inc. seeks a PATH Case Manager to provide outreach to homeless individuals and those at risk, connecting them with housing, mental health services, Medicaid, SNAP, and SOAR.

The role involves documenting interventions in HMIS, collaborating with community partners, and assisting clients in accessing entitlements and benefits. Bachelor's degree required; OBHP certification within 3 weeks.

Qualifications

• Bachelor's Degree is required.
• Previous related experience in mental health or related discipline preferred; community outreach and work with vulnerable populations is a plus.
• OBHP educational requirements must be completed within 3 weeks of hire.

Responsibilities

• Provides street outreach.
• Work with community partners to provide services to homeless individuals.
• Provides documentation of interventions per department standards.
• Drives and transports clients as needed for PATH related business.
• Data entry into HMIS.
• Apply for social security benefits through SOAR.
• Other duties as assigned.
