Upward Health, Inc. in Santa Fe, NM is seeking a Community Outreach Specialist who will educate and enroll potential patients in our in-home, whole-person care program.

You will manage a personal caseload, primarily via phone outreach with in-person visits as needed, build trust, and collaborate with the Outreach Manager to grow patient engagement across our services. • Proficient in using computer systems for documentation, communication, and managing outreach activities.
• Flexible and adaptable to a variety of outreach methods, including phone, in-person meetings, and mailings.
• Fluent in English; Spanish proficiency is a plus.
