Job summary

Upward Health is seeking a Community Outreach Specialist to establish our presence in the Las Cruces area. You will educate potential patients about our in-home, whole-person care model and guide enrollment into programs, primarily via phone outreach with some in-person visits.

In this role you’ll manage a personal caseload, collaborate with the Outreach Manager, and use community-based strategies to reach diverse populations while building trust and ensuring a smooth onboarding experience.

Qualifications

• Excellent verbal communication and interpersonal skills.
• Ability to manage a personal caseload efficiently.
• Fluent in English; Spanish proficiency is a plus.

Responsibilities

• Educate patients about Upward Health’s offerings, engage them in conversations, and facilitate enrollment into programs.
• Manage a personal caseload, primarily via phone outreach, with in-person visits as needed.
• Collaborate with the Outreach Manager and team to ensure successful patient outreach and onboarding.
• Help patients understand the full range of services and take the first steps toward improving their health.
