Job summary

Upward Health, Inc. is an in-home, multidisciplinary medical group delivering 24/7 whole-person care for physical, behavioral, and social health needs.

The Community Outreach Specialist will be the first contact for individuals seeking services, educating them about offerings and guiding enrollment through phone outreach, in-person visits, and community activities. The COS will manage a personal caseload, report to the Outreach Manager, and work closely with the team to ensure successful

• Experience in community outreach and patient enrollment is expected.
• Excellent communication and relationship-building skills.
• Ability to manage a personal caseload and meet outreach goals.

Responsibilities

• Educate potential patients about Upward Health’s services.
• Engage individuals in meaningful conversations and facilitate enrollment.
• Manage a personal caseload with phone outreach and in-person visits as needed.
• Collaborate with Outreach Manager and team to ensure outreach success.
