Job summary

Upward Health is seeking a Community Outreach Specialist to educate patients about our in-home, multidisciplinary care options and enroll them into programs. The COS will manage a personal caseload, using phone outreach, in-person visits, and other community strategies to engage patients.

You will work to ensure patients understand all services, build trust, and coordinate with the Outreach Manager to support program success in a fast-paced, community-driven environment.
