Job summary

Catholic Charities of the Archdiocese of Milwaukee in Wisconsin seeks an Outreach Case Manager to empower clients to become more self-sufficient by helping them prioritize needs and create sustainable plans.

You will develop individualized care plans, assist enrollment in Medicare, food assistance, disability programs, advocate for clients, monitor progress, maintain records, and collaborate with community partners.

Qualifications

• Bachelor's degree in social work or a related field (strongly preferred).
• Two years of experience in case management, family services, or social services is ideal.
• Travel required.

Responsibilities

• Develop individualized care plans based on client needs assessments, identifying areas for financial, physical, and emotional support.
• Assist clients in enrolling in services like Medicare, food assistance, disability programs, and more.
• Advocate for clients by connecting them to appropriate resources during times of crisis and providing ongoing personal support.
• Monitor client progress and adjust care plans as needed to ensure positive, sustainable outcomes.
• Maintain accurate case records, logging significant events and tracking client progress.
• Collaborate with internal programs and external community partners to provide comprehensive care and ensure clients receive all necessary services.
