Job summary

Catholic Charities of the Archdiocese of Milwaukee is seeking an Outreach Case Manager to help clients become more self-sufficient by creating sustainable care plans and connecting them to vital services. You will coordinate with internal programs and community partners to deliver comprehensive support.

The role requires a Bachelor's degree in social work or related field (strongly preferred), 2 years of case management experience, and travel with a valid driver's license.

Qualifications

• Bachelor's degree in social work or related field (strongly preferred).
• Two years of experience in case management, family services, or social services is ideal.
• Travel required — Valid driver's license, reliable transportation, and vehicle insurance.
• Bilingual in English and Spanish is strongly preferred.

Responsibilities

• Develop individualized care plans based on client needs assessments, identifying areas for financial, physical, and emotional support.
• Assist clients in enrolling in services like Medicare, food assistance, disability programs, and more.
• Advocate for clients by connecting them to appropriate resources during times of crisis and providing ongoing personal support.
• Monitor client progress and adjust care plans as needed to ensure positive, sustainable outcomes.
• Maintain accurate case records, logging significant events and tracking client progress.
• Collaborate with internal programs and external community partners to provide comprehensive care and ensure clients receive all necessary services.
