Job summary

Eau Claire County is seeking a Community Outreach Case Worker to provide support, guidance, and resources to individuals facing homelessness and related challenges. You will engage with clients, assess needs, and connect them to housing, mental health, and social services, emphasizing empowerment and self-advocacy.

Qualifications

• Bachelor’s degree in social work or related field.
• Experience with homelessness, mental health challenges, substance use, disabilities, or incarceration.
• 1 year of experience in family, children, or adult services or internship in relevant field.
• Conditional appointment after caregiver and criminal background checks.
• Valid driver’s license, acceptable driving record, and access to private transportation.
• Maintain personal automobile insurance as required.

Responsibilities

• Conduct outreach across Eau Claire County in shelters, hospitals, libraries, and events to engage individuals.
• Establish rapport with individuals through non-judgmental, empathetic interactions.
• Provide needs assessments and create personalized service plans with follow-up.
• Assist clients in housing, mental health, substance use recovery, and wellness goals.
• Connect clients to housing, mental health care, and employment resources.
• Document outreach activities and progress while complying with policies.
