Responsibilities

• Receives calls about Home Care clients and services and provides follow up.
• Routes calls, takes messages, and completes initial skills training for CDC program participants.
• Assists with enrollment paperwork for CDC program and conducts home visits.
• Mails notices and paperwork, contacts providers, and runs quality control reports.
• Participates in team meetings and supports home care staff with office tasks.
