Job summary

ADP, Inc. in Fall River, MA, is seeking a Geriatric Support Services Coordinator to perform initial member assessments, reassessments, and ongoing case management for SCO and One Care members in compliance with regulatory and agency guidelines.

The role coordinates with SCO/One Care partners, assists with public benefits applications, documents activity, and participates in interdisciplinary meetings to support member choice and independence.

Qualifications

• Bachelor’s degree in human services or related discipline.
• Two years working with disabled population and/or elders.

Responsibilities

• Conduct initial assessments to determine members’ needs and develop service plans.
• Assist with applications for public benefits and problem resolution related to benefits.
• Perform ongoing case management, including timely reassessments and adjusting service levels.
• Coordinate safe hospital and nursing facility discharges as needed.
• Maintain documentation in line with AGE, SCO, One Care, and agency requirements.
• Participate in interdisciplinary meetings and communicate with Care Team.
• Provide skills training for consumers enrolled in the personal care attendant program.
