Job summary

Family Health Center in Worcester, MA is seeking a Community Health Worker to support the Psychiatric Team, outreach and recruit patients for care and care management services, and educate and advocate to ensure access to FHCW resources.

The role requires bilingual ability, a high school diploma (Bachelor’s preferred), and CHW Core Training within the first year. You will coordinate referrals using Healthify and E-referral, document services, and help remove barriers to care.

• Provides outreach and meets with patients to explain services and coordinates the treatment plan.
• Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools.
• Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.
• Maintains strong organizational and communication links across multiple departments.

Salary Range: $20.67 To $26.92 Hourly

This position will support the Psychiatric Team.

Position Summary:

The Community Health Worker will support the Psychiatric Team to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services.

Program Specific Essential Duties and Responsibilities:

• Supports the care team with patient follow-up, reminders, and addresses barriers to care.
• Document all services as required and will assist in meeting all program requirements.
• Collects and analyzes data related to the project and works with data management staff for data collection.
• Provides outreach and meets with patients to explain services and coordinates the treatment plan.
• Facilitates referral process for patients using Healthify, E-referral, Community Help, and other tools as applicable.
• Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.
• Strong organizational and communication skills to act as a hub of information across multiple departments.
• Any and all other duties as assigned.

Qualifications and Education Requirements:

Minimum of two years relevant work or college-level internship experience required. Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners.
