• Bilingual in English and another language appropriate to patient population.
• CHW Core Training within first year of employment.

Responsabilités

• Support the Psychiatric Team with outreach, recruitment, and patient engagement in care and care management services.
• Provide patient education and advocacy to access services.
• Document services and assist in meeting program requirements.
• Coordinate referrals and follow-ups using Healthify, E-referral, and other tools.
• Advocate for transportation, interpretation, and health insurance for patients.
