Job summary

COPE Health Solutions ECM Program in Merced, CA seeks a Community Health Worker to educate patients about ECM services, assist enrollment, and serve as the primary liaison between members and required services. You will support families navigating the health care system and transition to improved self-care and health management.

Responsibilities include establishing trusting relationships, providing follow-up and motivational interviewing, removing access barriers, and coordinating with the care

Qualifications

• High school graduate or equivalent; associate degree preferred.
• Bilingual English/Spanish preferred.
• EMR experience preferred.
• CA driver’s license required; clean driving record preferred.
• Experience in a community setting 1–2 years preferred.

Responsibilities

• Educate members about ECM services and assist with enrollment; liaison between member and services.
• Support navigation of health care system and transition to self-care and management.
• Establish trusting relationships with patients and families; provide support and encouragement.
• Provide follow-up, motivational interviewing, and goal setting with patients/families.
• Facilitate appointments, assess care gaps, and connect with community resources.
