Job summary

Comprehensive Community Health Centers, Inc. seeks an ECM Community Health Worker to support patients with complex medical and social needs. You will engage patients through outreach, home visits, and community events, helping them access health services and community supports.

You will build trust, educate patients, and coordinate with care teams to reduce barriers and improve outcomes. Fluency in Spanish or Armenian is required, and prior CHW experience is preferred.

Qualifications

• Bilingual Spanish or Armenian required.
• Associates or Bachelor’s degree preferred.
• CHW/P/R Certification preferred; not required
• 1+ years working in related field preferred.
• High school diploma or GED required; Associate’s or Bachelor’s degree in Public Health, Social Work, Human Services, or related field preferred.
• Minimum 1–2 years of experience in community outreach, patient navigation, health education, or related field.

Responsibilities

• Conduct outreach to patients eligible for ECM services via phone, home visits, and community engagement.
• Build trusting relationships with patients, families, and caregivers to support care plan goals.
• Assist patients in navigating healthcare systems, including appointments, transportation, and access to primary/behavioral health services.
• Connect patients to community resources (housing, food, financial assistance, social services).
• Support care managers by reinforcing care plans and helping patients follow through on goals.
