Job summary

COPE Health Solutions is seeking a Community Health Worker to help patients and families navigate services, enroll in programs, and coordinate care with the ECM team. You will build trust, offer basic counseling, and assist in removing barriers to accessing health and social services.

This full-time, on-site role serves San Pedro and Whittier, emphasizing outreach, home visits, and collaboration with physicians.

Qualifications

• High school diploma or equivalent required; Associate's Degree preferred.
• Fluency in English and Spanish preferred.
• Basic computer skills; EMR experience preferred.

Responsibilities

• Educating members about ECM services, assisting with enrollment and serving as liaison between member and services.
• Support individuals and families to navigate the health care system and improve self-care and management.
• Establish trusting relationships with patients and families while providing support and encouragement.
• Provide follow-up, motivational interviewing, and goal setting with patients/families.
• Bridge conversations with members to remove barriers to health and social services; conduct face-to-face outreach for appointments and care gap closure.
• Meet members in clinic, facility or at home to identify social determinants of health affecting well-being.
• Collaborate with the care team to create individualized, culturally appropriate care plans.
• Assist with accessing health-related services and community resources; accompany to appointments and help with enrollment forms.
