Quinault Wellness Center seeks a compassionate Community Based Worker to help individuals navigate healthcare services and access community resources. The role involves identifying needs, coordinating referrals, and supporting patients through housing, food, transportation, and insurance programs.

You will work with a multidisciplinary team to connect patients with appropriate services, maintain documentation, and follow up to ensure ongoing engagement in care.

Competenze

• Experience working in healthcare, behavioral health, social services, community services, or a human services environment.
• Ability to research, identify, and connect individuals with appropriate community resources and assistance programs.
• Knowledge of healthcare systems, community resources, and PHI/HIPAA requirements.
• Excellent communication and interpersonal skills with the ability to build trusting relationships with patients, families, and healthcare providers.
• Ability to navigate applications, referrals, appointments, and resource programs.

• Identify individual needs and connect patients with appropriate community resources and support services.
• Assist patients with navigating applications, referrals, eligibility requirements, and available programs.
• Research and identify local, regional, and statewide resources to address patient needs.
• Coordinate housing, food, transportation, insurance, financial support, and other social services for patients.
• Coordinate appointments, referrals, and follow-up to keep patients connected to healthcare providers.
• Provide outreach and follow-up to encourage patient engagement and overcome barriers to care.
• Accompany or transport patients to appointments, referrals, and community resources when appropriate.
• Build relationships with community agencies and resource providers to expand support available to patients.
• Collaborate with medical, behavioral health, dental, pharmacy, and substance use disorder teams to coordinate patient support.
• Maintain accurate documentation of patient interactions, referrals, and follow-up activities.
