Job summary

Oak Street Health is seeking a Community Health Worker to bridge patients, families, and medical systems, promoting health literacy and access to resources. You will navigate social determinants of health, coordinate with medical social workers, and engage communities through in-person outreach and strong partnerships.

Your role emphasizes cultural competency, transportation logistics, and compassionate support to improve quality of life.

Qualifications

• Minimum 1 year of experience in healthcare, community-based, case management, or social service environment.
• Strong oral and written communication skills.
• Ability to manage multiple priorities while maintaining a positive attitude.
• Dedication to serving the community and building meaningful relationships.
• Proficient computer skills (Windows, GSuite, Microsoft).
• Access to reliable transportation and ability to travel throughout the community.

Responsibilities

• Establish and maintain relationships with patients, community organizations, team members, and partners to coordinate patient needs.
• Manage patient referrals defined by the care team and collaborate with the Medical Social Worker on action plans.
• Facilitate communication among patients, families, caregivers, providers, and community organizations.
• Develop relationships with local community organizations to benefit patients.
• Connect patients to community resources for housing, transportation, food, and daily living needs.
• Assist patients with applications for accessing eligible benefits and resources.
• Document interactions in the electronic medical record in a timely manner while upholding HIPAA standards.
• Plan time effectively, work independently, and collaborate with an interdisciplinary medical team.
