Formación

• High School diploma or GED required.
• Experience in healthcare, behavioral health, social services, community services, or human services.
• Knowledge of healthcare systems and community resources.
• Ability to research, identify, and connect individuals with appropriate resources and assistance programs.
• Strong communication and interpersonal skills, with the ability to build trusting relationships with patients and partners.
• Ability to navigate applications, referrals, appointments, and resource programs.
• Ability to work with diverse backgrounds and cultures.
• Understanding of HIPAA and patient confidentiality requirements.
• Ability to coordinate multiple referrals, appointments, and follow-ups.
• Valid driver''s license and insurability.

Responsabilidades

• Identify patient needs and connect patients with appropriate community resources and support services.
• Help patients navigate applications, referrals, eligibility and available assistance programs.
• Research and identify local, regional, and statewide resources to address patient needs.
• Assist patients with housing, food, transportation, insurance, financial support, and other social services.
• Coordinate appointments, referrals, and follow-up services to keep patients connected to care.
• Provide outreach to encourage patient engagement and overcome barriers to care.
• Accompany or transport patients to appointments and community resources when appropriate.
• Build relationships with community agencies and social service organizations.
• Collaborate with medical, behavioral health, dental, pharmacy, and SUD teams to coordinate support.
• Maintain accurate documentation of patient interactions and referrals.
