Responsibilities

• Advocate for patients and coordinate referrals with dental and medical providers
• Provide healthcare navigation, insurance enrollment assistance, and care coordination
• Maintain accurate documentation in patient charts and tracking systems
• Develop and maintain community partnerships and outreach activities
