Job summary

Union Community Care seeks a Community Health Resource Coordinator to help patients meet social and behavioral goals. You will work with medical, dental, and behavioral teams to coordinate social services and enroll families in health insurance programs, providing language-accessible support and advocacy across community partners.

Qualifications

• Bachelor's degree preferred; HS diploma required.
• 3+ years of relevant experience.
• Certified Application Counselor or Navigator required.
• Ability to communicate effectively in person and in writing and collaborate with community organization representatives, health center staff, and clinicians.
• Ability to document and schedule in the Electronic Health Record.
• Ability to engage effectively at outreach events in the community.
• English proficiency required.

Responsibilities

• Act as a medical and dental team member to coordinate social services referrals and ensure integrated care.
• Collaborate with individuals and families to create care plans addressing social determinants of health.
• Promote social assistance including health insurance, housing, mental health referrals, and public benefits.
• Enroll adults, children, and families in health insurance programs.
• Conduct outreach to promote health insurance for children, adults, and families.
• Provide services in the patient’s preferred language.
• Advocate between patients and other providers.
• Share best practices with coordinators and organization-wide.
• Represent Union Community Care in public venues.
• Meet with families regularly to remove barriers and plan next steps.
• Document referrals and patient communications in the EHR.
• Develop relationships with local organizations for referrals.
