Job summary

McLeod Health is seeking a Community Care Navigator to help coordinate access to health care services for low income and underinsured clients within our Access to Health programs.

You will assess social determinants of health, facilitate referrals, and support patients in navigating health resources. Role involves home visits, community outreach, and collaboration with care teams to establish medical homes and reduce unnecessary ER visits.

Qualifications

• Strong knowledge of community resources and services.

• Basic computer skills, including familiarity with Microsoft Office and Excel.

Responsibilities

• Assist with coordination of access to health care services and provider referrals to eligible clients.
• Assess social drivers of health and facilitate access to care.
• Provide navigation and support to establish a medical home and coordinate care.
• Engage with community members through home visits, meetings, and outreach.
• Liaise between patients and healthcare providers to ensure coordinated care plans.
