Job summary

Enhabit Home Health & Hospice is seeking a skilled relationship manager to build and nurture partnerships with physicians, hospitals, and other referral sources in Colorado. This role focuses on growing referrals and educating partners about Enhabit's services to support seamless post-acute care transitions.

You will engage patients, families and care teams to promote high-quality outcomes while navigating regulatory guidelines and maintaining strong professional relationships.

Qualifications

• Degree in healthcare-related field or nursing/therapy/social work background.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal, state, local Medicare rules for home health and hospice.

Responsibilities

• Manage relationships with referral sources including physicians, hospitals, and health systems.
• Educate referral sources on services and program requirements to grow referrals.
• Support post-acute care transitions to improve patient outcomes and satisfaction.
