Job summary

Enhabit Home Health & Hospice seeks a skilled professional to manage key referral-source relationships and educate partners on our programs. You will support post-acute care transitions, aiming to improve patient outcomes and satisfaction, while expanding referral networks.

The role requires a healthcare background, strong communication and presentation skills, and a valid driver’s license. In-office presence is essential to achieve objectives and collaborate with teammates.

Qualifications

• Bachelor’s degree in healthcare administration, marketing, or related field, or nursing/therapy/social work degree.
• Strong interpersonal, communication, and presentation skills.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of federal/state/local laws and Medicare home health regulations.
• Valid state driver's license and automobile liability insurance.
• Clinical license if applicable.

Responsibilities

• Serve as primary relationship manager between the company and referral sources (physicians, hospitals, health systems, and others).
• Promote services and educate referral sources about program requirements to support post-acute care transitions.
• Collaborate to improve patient outcomes and satisfaction with care transitions.
• Support referral growth and nurture partnerships with community providers.
