Job summary

Enhabit Home Health & Hospice is seeking a skilled relationship manager to serve as the primary liaison with physicians, hospitals, and referral sources. You will educate partners about Enhabit’s services and programs, support referral growth, and navigate post-acute care transitions to improve patient outcomes.

Ideal candidates bring 2+ years in healthcare business development or sales, strong communication and presentation skills, and a valid driver's license.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field, or related nursing/therapy/social work program.
• Minimum two years' experience in healthcare business development, community relations, marketing, or sales.
• Understanding of Medicare home health operations and regulatory guidelines.
• Valid driver's license and reliable transportation.
• Clinical licensure if applicable; otherwise non-clinical experience acceptable.

Responsibilities

• Serve as the primary relationship manager with referral sources.
• Promote awareness and education of company services and program requirements.
• Support referral growth and navigate post-acute care transitions.
• Help improve patient outcomes and satisfaction through effective care transitions.
