Qualifications

• Must have strong interpersonal, communication and presentation skills.

Responsibilities

• Acts as the primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, senior living communities, and other networks).
• Promotes awareness of Enhabit services and program requirements to referral sources, patients, families, and caregivers to support post-acute transitions and improve patient outcomes.
