Job summary

Enhabit Home Health & Hospice is seeking a proactive relationship manager to cultivate referrals with physicians, hospitals and health systems. You will educate partners on Enhabit services, support post-acute transitions, and strive to improve patient outcomes and satisfaction.

The role requires driving for referrals, strong communication, and knowledge of Medicare/home health regulations. In-office work is required, with competitive benefits and opportunities for growth.

Qualifications

• Bachelor's in healthcare administration, marketing, or related field or equivalent work experience.
• Graduate of an approved nursing, therapy, or social work program is highly preferred.
• Minimum two years in healthcare business development, community relations, marketing or sales.
• Knowledge of Medicare home health and hospice regulations is required.
• Valid state driver's license and automobile liability insurance are required.
• Strong interpersonal, communication and presentation skills.
• If clinical: must hold a clinical license per state rules.
• Experience in home health, hospice or post-acute care is preferred.
• Experience with electronic referral systems and Homecare Homebase (HCHB) is a plus.
• Established relationships with physicians, hospitals or senior communities are a plus.
• Reliable transportation and the ability to drive in various weather conditions.

Responsibilities

• Acts as the primary relationship manager with referral sources including physicians, hospitals and health systems.
• Educates referral sources about Enhabit services to grow referrals and improve patient outcomes.
• Supports post-acute care transitions to optimize care and patient/referral source satisfaction.
