Job summary

Malama Health is seeking a Community Doula in Michigan to serve as a Maternity Care Navigator using our tools to support pregnant and postpartum mothers. You will bridge gaps between providers, community organizations, and patients, ensuring adherence to care protocols and equity in access to services.

The role emphasizes outreach, relationship-building, and travel across counties, with training and growth opportunities within our fast-growing healthcare technology startup.

Qualifications

• 1–2 years of experience in Community Health/Care Coordination roles.
• Certified birth doula with CPR training and births experience.
• Fluent in English; bilingual in Spanish preferred.
• Located in Michigan with ability to travel across counties.

Responsibilities

• Deliver Care Management: outreach, check-ins, assessments via phone, SMS, video, or in person.
• Coordinate care services and referrals with health plans and providers to drive services to completion.
• Communicate with patients using motivational interviewing and trauma-informed care.
• Document and track care coordination activities in portals; manage caseloads and follow-ups.
• Conduct in-person and virtual outreach at clinics and community sites; staff events and give-aways.
