Moms In Motion is seeking a Service Facilitator (SF) Case Manager to visit families in their homes across Virginia, helping them access Medicaid Waiver programs and coordinate care.

You will travel locally, document plans of care, and support families to live safely and independently. No degree is required, and strong communication and tech skills are essential. The role offers comprehensive benefits and a pathway to make a tangible difference in communities.

Qualifications

• Minimum 2 years of experience in disability or elder care.
• No degree required.
• Strong written and verbal communication.
• Valid driver’s license and reliable vehicle.
• Able to pass background check and provide 2 professional references.

Responsibilities

• Visit clients in their homes for assessments.
• Assist families navigating VA Medicaid Waiver programs.
• Write plans of care and assessments.
• Troubleshoot service authorizations and timesheets.
• Build lasting relationships with families based on respect, patience and trust.
• Work remotely from your laptop/tablet.
