Qualifications

• Bachelor’s degree in human services or a related field.
• At least two years of general work experience providing support to individuals with behavioral health services.
• Intermediate computer skills.
• Valid driver’s license.
• Ability to travel throughout the program’s service-delivery area.
• Ability to transport program participants to appointments as required.
• Preferred: at least one year of case management experience.

Responsibilities

• Participate as a member of a multidisciplinary team providing holistic, values-based services.
• Assist with development and review of individualized recovery plans with participants and staff.
• Help determine resources and services to support participants’ goals.
• Advocate for program participants to access appropriate services and supports.
• Develop and maintain relationships with community agencies.
• Provide community-based services and educate participants about available resources.
• Complete assessments and documentation in a timely manner.
