People Encouraging People in Baltimore, MD, is seeking a dedicated BCORE Case Manager to provide case coordination for unhoused individuals and those with behavioral health disorders. The role involves coordinating care between the Agency, community and home settings, with a focus on housing services and crisis intervention.

The ideal candidate has 5+ years in case management, a valid driver's license, and comfort using HMIS and related databases.

Qualifications

• 5+ years of experience providing case management to vulnerable populations.
• Education: High School Diploma and/or equivalent work experience; bachelor’s or associate degree preferred in social services.
• Valid Driver’s License and use of personal vehicle.

Responsabilités

• Screen clients upon admission and complete intake packets for services.
• Meet clients to evaluate needs and create a case plan.
• Coordinate with agencies and refer clients to housing, treatment facilities or resources.
• Monitor progress, update treatment plans, and ensure goals are met.
• Maintain quality, confidential documentation per policies and procedures.
