Job summary

Bay Area Community Services seeks a Care Coordinator/Case Manager to provide high-quality therapeutic interventions and case management for individuals in recovery. The role includes housing navigation, crisis intervention, and family support, with duties in office and community settings in San Mateo County.

The position requires 4 years of relevant mental health/social services experience, a valid CA driver’s license, and willingness to travel extensively.

Qualifications

• Experience in mental health or social services is required.
• Customer service experience may count toward the required years.
• Must possess a valid driver’s license and access to a personal vehicle.

Responsibilities

• Develop and deliver a full complement of services including counseling and case management for assigned individuals.
• Use evidence-based practices to ensure high-quality outcomes.
• Assess risk status and intervene appropriately based on symptoms, substance use, environment, and health.
• Create service plans focused on recovery and client-centered goals.
• Engage with outreach and determine when to disenroll per program protocols.
• Coordinate care planning and therapeutic case management using a flexible approach.
• Maintain data, reporting, and charting in compliance with policies; meet service-unit targets.
• Participate in supervision, meetings, travel, and training as required.
