Job summary

PEP is seeking a BC ORE Case Manager in Baltimore, MD to provide case coordination for unhoused individuals and those with behavioral health disorders. The role involves coordinating care between the Agency, community, and home settings with a focus on housing services.

The ideal candidate has 5+ years of case management experience, a valid driver's license, and the ability to navigate social services networks.

Qualifications

• Experience in case management for vulnerable populations.
• Proficient in HMIS and client data documentation.
• Strong communication and problem-solving skills.
• Valid driver's license and access to a personal vehicle.

Responsibilities

• Screen clients upon enrollment and complete intake packets.
• Meet clients to assess needs and develop case plans.
• Coordinate with community resources and treatment facilities.
• Monitor progress and update care plans as required.
• Maintain confidential records per policy and procedures.
• Educate clients and families about available resources and recourses.
• Provide advocacy and crisis intervention when needed.
• Attend community meetings and participate in care conferences.
• Visit agencies, shelters and hospitals to assess housing needs.
