Job summary

Care Lync in Conway, SC, is seeking a dedicated Support Coordinator/Service Coordinator/Case Manager to develop personalized care plans and coordinate services for adults with IDD, autism, and physical disabilities enrolled in Medicaid waivers. You will broker services, monitor outcomes, and help clients access housing, medical and community supports.

Travel up to 50% may be required as part of coordinating with providers and assessing participant needs to ensure comprehensive, cost-effective

Qualifications

• BS/BA degree in related field.
• Minimum of 3 years related experience providing case management for individuals with disabilities.
• Experience with Microsoft Office and state-maintained databases.
• CPR/First Aid certification.
• Valid Driver's License with insurance and current vehicle registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update as needed.
• Ensure participant chooses providers with informed decisions.
• Coordinate with care teams to ensure seamless service delivery.
• Maintain confidentiality of sensitive information and complete required trainings.
• Travel up to 50% to participants as needed.
