Job summary

AWARE is seeking a client access care coordinator to join our team in Montana. The role focuses on helping adults, children, and families access coordinated care and community resources with a compassionate, client-centered approach.

You will receive referrals, assist with Medicaid and VA enrollment, and develop care plans in collaboration with clients, families, and care teams. We offer a competitive benefits package, opportunities for professional growth, and a supportive, inclusive work

Qualifications

• High School Diploma or Equivalency.
• One year of direct work experience.
• Valid Montana Driver’s License or willingness to obtain one.
• Must have acceptable driving record and pass a criminal background check.

Responsibilities

• Receive referrals and identify client needs (Medicaid/Veterans and interagency services).
• Assist clients with payer enrollment (Medicaid/Veteran Affairs).
• Complete care coordination assessment and develop a client-centered plan with the consumer, family/caregiver(s) and providers.
• Plan and monitor care to assess unmet health and social needs.
• Identify high-risk consumers and respond to urgent needs.
• Communicate with care agencies regarding client needs and resources.
